Existing pets have either 1) already had
at least one Lab Requisition form created, or 2) been uploaded with
active clients from the hospital’s veterinary software. Pet
name and species are typically already in the database for existing
patients. When you access an existing pet for the first time, you
will need to provide breed and sex information. This information
will then be stored in your Lab Ordering database
To access an existing pet, first locate
the pet owner. Then, select the pet name from the drop-down list,
as shown below.

Always check existing pets’ species,
breed, and sex information to ensure that it is correct, as it is
this information that will be printed on the Lab Requisition form.
The following fields are MANDATORY: Client Name, Pet Name, Species,
Breed, Sex and Age. If any of this information is omitted, an error
message will be returned when the Lab Requisition form is submitted.
To select species, breeds, and sex, click
the drop down lists for each item. You can also type the letter
that the species, breed or sex begins with, and continue typing
the same letter to scroll through options (for example, in the breed
field, for canines, typing D repeatedly will display Dachshund,
Dalmatian, Doberman, etc.) You can click with your mouse or TAB
to navigate between the fields.
Note: There are FIVE selections
in the drop-down list for sex: Male, Female, Castrated Male, Spayed
Female, and Unknown. To receive accurately reported results, it
is important to choose the most correct of these selections.
