Once you have completed the interactive Lab Requisition form, you will want to generate the bar-coded test request page that will be included in your sample submission bag. To generate this form, click on the red CONTINUE button that can be found on the top left hand side of the page.

A new window will open containing the bar-coded lab requisition form.
