Using the Histo-Cyto Lab Requisition Form
   
 

To submit a histology or cytology, you must use the purple Histo-Cyto form.

 

1. You can request up to three different pathologists to direct your specimen to. Not all pathologists are available at all times, however Antech will make every effort to direct your samples according to your preferences. You can find this option at the top right of the Histo-Cyto form.

2. To indicate the location of a performed biopsy, circle it once you have printed off the form

3. You must supply information about the biopsy type, from one of the following four choices: Excisional, Incisional, Needle or Endoscopic.

4. You MUST supply a patient history. A bar-coded Lab Requisition form cannot be generated if the patient history is omitted.

5. If the sample is critical, make sure to select STAT next to the test you are requesting. STAT requests are not always possible and may require an additional fee for the priority service provided by Antech Diagnostics.
