The Lab Requisition form is a computer-generated lab request form, which must be included in the specimen bag for lab pickup. It includes a bar-code, which ensures accurate entry of all patient and hospital information once it reaches the lab. A Lab Requisition form is shown below. To print the Lab Requisition form, click the RED Print button. Click "New" to complete the lab requisition form and return to the Zoasis lab module to create another lab requisition form.
