If your hospital submits samples to more than one lab (for example, Antech West and Antech East), you will need to select which lab you are submitting your sample to. To select a lab, click on the drop-down box next to Lab Account #. Click to select the appropriate choice. You must select a lab. If no lab is chosen, an error will occur when a Lab Requisition Form is selected.
